An Exciting New Avenue For Epilepsy Treatment Development


A ketogenic diet, high in fat and low in carbohydrate, has been used in the treatment of drug-resistant (refractory) childhood epilepsy since the 1920s. It forces the body to burn fats instead of carbohydrates, and this leads to a build-up of molecules called ketone bodies in the blood, which are thought to be responsible for the diet’s anti-convulsant effect in some people.
The ketogenic diet does, however, have a number of unpleasant side-effects, such as constipation, bone fracture and delayed growth; and it is generally not well tolerated by adults (who find it difficult to persevere with). Research efforts are therefore ongoing to gain a better understanding of the anti-convulsant mechanisms of the diet, so that a drug that has its benefits but avoids its side-effects can be developed to replace it. Such a treatment might also be a viable option for adults with refractory epilepsy.
Clinical studies have now shown that there is, in fact, no link between blood ketone body levels and seizure control; and this suggests that other factors must be at the root of the diet’s anti-convulsant effect. Researchers at Royal Holloway University London (RHUL) have recently made exciting progress in identifying these.
In their latest study, the team focused on molecules known as medium chain fatty acids (MCFAs), which also accumulate in the blood during the the ketogenic diet. Fatty acids are the building blocks of fats, and there are many different types depending on their structure. The reason the scientists were interested in MCFAs is because valproate (a widely prescribed anti-epileptic drug) is also a type of fatty acid; although it is a short-chain fatty acid, not a medium chain one.
Valproate has a number of unpleasant side-effects, and it poses considerable risks to the development of unborn babies if their mothers take it during pregnancy (this is referred to as teratogenicity). MCFAs are significantly different to short-chain fatty acids in terms of their size and structure, and the scientists hoped that they would find at least one that could stop seizures as effectively as valproate (if not more) AND have fewer side-effects.
The group began by identifying three MCFAs: two that are known to accumulate in the ketogenic diet (ethanoic acid and decanoic acid), and one that is also found naturally but does not tend to increase during the diet (nonanoic acid). They tested each one (at a range of doses) using cell cultures to find out i) their ability to stop seizure activity ii) whether they were toxic to liver cells and iii) their teratogenic risk, and then compared them to valproate.
The results of the first assessment showed that decanoic acid and nonanoic acid were superior to valproate in stopping experimental seizures (they required a lower dose to achieve the same effect); but this was not the case with ethanoic acid. When an ethanoic acid derivative called 4-methylethanoic acid was tested, however, it too out-performed valproate. This compound was therefore included in subsequent assessments.
Liver toxicity is notoriously difficult to assess in cell cultures, and whilst none of the molecules showed significantly greater toxicity compared to valproate, it was not easy to determine from the results whether any were less toxic.
In terms of teratogenicity, both nonanoic acid and 4-methylethanoic showed minimal risk (at all doses tested); however decanoic acid appeared to be as teratogenic as valproate at some clinical doses.
After considering these findings and weighing the benefits of each MCFA against their potential side-effects; the researchers decided to take 4-methylethanoic acid and nonanoic acid forward for further testing, in animals. Here, they assessed the tendency of each to cause drowsiness/sedation (which can be extremely debilitating to daily life) and their ability to protect neurons from damage caused by status epilepticus (SE), and again they compared them to valproate. Interestingly they found that nonanoic acid caused less sedation than valproate in the animals, but that with 4-methylethaonic acid, sedation was more pronounced. Furthermore, neither 4-methylethanoic acid nor valproate protected the brain from neuronal damage during SE, whereas in animals treated with nonanoic acid, this damage was markedly reduced.
These results are very promising, because they pinpoint nonanoic acid as a molecule that is superior to valproate in terms of seizure control and side-effects, and suggest that it might also offer protection to neurons during seizures. In time, drugs that mimic nonanoic acid’s function could potentially replace the ketogenic diet and become a viable treatment for both adults and children. They may even prove to be safer and more effective than some existing anti-epileptic drugs in certain types of epilepsy.

Empiric Use of Potassium Citrate Reduces Kidney-Stone Incidence With the Ketogenic Diet



For nearly a century, the ketogenic diet (KD) has been recognized as an effective nonpharmacologic treatment for epilepsy. Despite positive effects on seizure frequency, repeated studies have shown that the KD is associated with an increased prevalence of kidney stones, ranging from 3% to 6% during a typical 2-year diet duration. Hypercalciuria, urine acidification, and hypocitraturia, all known risk factors for kidney stones, often occur in patients who on the KD.3
In 2000, a study from our group identified that children who had hypercalciuria (urine calcium-to-creatinine ratio typically >0.2) and were on the KD were at highest risk for kidney stones. Past studies have shown that oral potassium citrate (Polycitra K, Cypress Pharmaceuticals, Madison, MS) effectively increases urinary pH, increases urinary citrate, and decreases incidence of kidney stones. From 2000 to 2005, our approach was to treat children on the KD at the time of laboratory-identified hypercalciuria with Polycitra K. In this group, hypercalciuria occurred in 101 (52%) of 195 patients, usually after 3 to 6 months.7 Unfortunately, the incidence of kidney stones did not decrease with this strategy, with 13 (6.7%) developing stones during this period; however, in 10 of these children, hypercalciuria was not noticed, was treated, or the child refused treatment with Polycitra K.
In light of the results from this study, we began empirically giving all children, regardless of hypercalciuria, Polycitra K at KD initiation. We believed that the benefits outweighed any risks and that universal use would avoid confusion and improve compliance. In this cohort study, we examined whether empiric Polycitra K administration could reduce the 6.7% incidence of kidney stones in children who are started on the KD.7

METHODS

Medical charts of all children who were started on the KD at Johns Hopkins Hospital from January 1, 2000, to December 31, 2008, and had at least 1 month of follow-up were analyzed retrospectively. After an initial 24- to 48-hour fast, patients' caloric intake was gradually increased at either a 3:1 or 4:1 (fat to protein or fat to carbohydrates, respectively) ketogenic ratio during a 3-day period.1 Children were provided a fluid allotment of ∼80% to 90% of their recommended daily requirements but often increased to 100%.1 Most children were rarely drinking their fluid requirements even before starting the KD, and the fluid allotment is often an increase over their baseline. Children were typically seen in the clinic every 3 to 4 months for the first year and every 6 months thereafter, at which time all medications, including Polycitra K, were confirmed with parents. The presence of a kidney stone was defined as (1) stones or stone fragments found to have been passed in urine or noted in the diaper, (2) gross or microscopic hematuria concurrent with lower flank pain, or (3) evidence on ultrasound or computed tomography scan of a stone or nephrolithiasis.
From January 2000 to December 2005, 195 children were administered the KD and treated with Polycitra K as described previously, treating only for identified hypercalciuria.7 This group is referred to as the reactive group. From January 2006 to December 2008, all children who started on the KD at our institution were started empirically on Polycitra K at hospital discharge and are further described as the empiric group.
Polycitra K is available in a powdered packet of 30 mEq that can be dissolved in fluid or sprinkled onto ketogenic foods such as heavy whipping cream or eggs, with a taste similar to lemons. The prescribed dosage of Polycitra K, 2 mEq/kg per d divided twice daily, remained consistent in both groups. In general, most young children (<20 kg) received half a packet twice daily for ease of administration, and older patients (>20 kg) received ∼1 packet twice daily. When a child refused, we recommended sodium citrate tablets (Bicitra, Ortho-McNeil-Janssen Pharmaceuticals, Titusville, NJ) instead.
Charts and electronic records were reviewed for patient age, gender, immobility (defined as spending the majority of their time in a wheelchair or stroller past 2 years of age), carbonic anhydrase inhibitor (topiramate or zonisamide) use, kidney-stone occurrence, and Polycitra K use for both groups. Urine pH and serum CO2 were analyzed after 3 months on the KD when available. Urine calcium and creatinine were obtained in the reactive group approximately every 3 to 6 months but no longer monitored regularly in the empiric group because it did not affect management or added cost, and our previous study showed no change in hypercalciuria with oral citrates.7Baseline urine pH was examined for the 50 most recent patients.
This study was approved by the Johns Hopkins institutional review board, and all families consented for their information to be included in a centralized database. Categorical data were analyzed using χ2 test with Yates correction. Means were compared using a paired 2-sample t test. Multiple regression analysis was used to adjust for carbonic anhydrate use and diet duration (in 12-month intervals) when examining for kidney-stone occurrence. The significance level for all tests was P = .05.

RESULTS

Overall Effects of Receiving Polycitra K

Between January 2000 and December 2008, 313 children with intractable epilepsy were started on the KD. Seven children discontinued the KD before 1 month, and 3 children were lost to follow-up; these cases were not included in the analysis. Polycitra K was administered preventively (either because of presence of hypercalciuria in the reactive group (n = 92) or at KD onset in the empiric group (n = 106) to 198 children at some point during their KD care, 4 (2.0%) of whom developed kidney stones. For children who did not receive Polycitra K, 11 (10.5%) of 105 developed kidney stones. Between the 2 groups, preventive use of Polycitra K decreased the incidence of kidney stones as compared with the group that did not receive Polycitra K (P = .003).
Overall, the median time to kidney-stone presentation was 6 months (range: 1–28 months). Only 3 stones were retrieved and analyzed; they were calcium oxalate, calcium carbonate, and uric acid in composition. Although there were sporadic reports about poor taste of the oral citrate powder, there were no adverse effects noted with its use.

Comparison of Reactive to Empiric Polycitra K Approach

From 2000 to 2005, 195 children were treated with the KD and are referred to as the reactive group. Of this group, 161 (83%) were appropriately treated only when hypercalciuria occurred as planned. For children who were not started on Polycitra K, hypercalciuria was never present, laboratory results were not available at clinic visits, or hypercalciuria was not treated because of neurologist or parent decision. In total, 92 (47%) children received Polycitra K in the reactive group, 10 directly as a result of a symptomatic kidney stone.
From 2006 to 2008, 108 children were started on the KD and are referred to as the empiric group. Of this group, 106 (98%) reported compliance with the recommended universal Polycitra K use (P < .001). Two children in the empiric group did not receive Polycitra K. One of these children, a 9-month-old boy, was not given Polycitra K because of parental confusion with provided prescriptions and developed hematuria and a kidney stone after 3 months. Symptoms resolved with extra fluids and Polycitra K, and he remained on the KD for a total of 17 months. A second child, a 4-year-old girl, has repeatedly refused Polycitra K because of taste according to her parents. She has not developed a stone or hypercalciuria to date and has remained on the KD for the past 18 months.
Table 1 summarizes the comparison between the reactive'(n = 195) and empiric (n = 106) approaches, excluding the 2 children in the empiric group who were not given Polycitra K. Empiric administration of Polycitra K at KD onset resulted in a kidney-stone prevalence of 0.9% compared with preventive administration (6.7%; P = .02). Only 1 child in the empiric group who was receiving Polycitra K developed a kidney stone. This 2-year-old girl developed a stone and hematuria after 5 months on the KD. She was not receiving any carbonic anhydrase inhibitor, and her parents reported daily use of Polycitra K as prescribed. Urine pH was 6.0 and serum CO2 was 13 mg/dL at the time of the stone. Lithotripsy was performed, and she has remained on the KD (and Polycitra K) 21 months to date without any additional stones.
Both diet duration and use of carbonic anhydrase inhibitor showed slight differences between the empiric and reactive groups. Mean diet duration in the empiric group was 13.3 months compared with a mean diet duration of 15.6 months in the reactive group (P = .11). Use of carbonic anhydrase inhibitor was 53 (50%) of 106 in the empiric group and 82 (42%) of 195 in the reactive group (P = .19). In a multivariate logistic regression that adjusted for both diet duration and carbonic anhydrase inhibitor use, the an odds ratio was 6.32 (95% confidence interval: 0.79–50.67; P = .08). The mean urine pH in the empiric group was higher (6.8 vs 6.2; P = .002), although there was not a similar increase in serum CO2. As a comparison, the baseline urine pH before starting the KD was 6.8 (SD: 0.9).

DISCUSSION

Overall, these results confirm previous evidence that Polycitra K administration is effective for kidney-stone prevention in children who receive the KD.7 The results are perhaps more convincing now than in our previous study, with a fivefold overall decrease in kidney-stone incidence with its use overall, compared with a threefold reduction previously.7 Polycitra K was well tolerated as a supplement, and no clear adverse effects were reported.
Comparing the 2 approaches of Polycitra K administration at our center, there was a sevenfold drop in the incidence of kidney stones with an empiric, universal treatment approach since 2006 (0.9% vs 6.7%). This approach is easier to follow because there is no longer any need for regular monitoring of the urine calcium-to-creatinine ratio. In addition, the ketogenic diet team can simply confirm with parents that oral citrates are being administered, similarly to multivitamins and calcium. Using Polycitra K from the start of the KD may also theoretically prevent stones from beginning to form, because no urine calcium-to-creatinine values were obtained until the 3-month clinic visit in the reactive approach. We are now recommending Polycitra K for all children who are in the reactive cohort (started on the KD before 2006) and still remain on the KD to date.
Relative costs need to be considered for any treatment, and the average cost for Polycitra K for the median 14 months in this study is approximately $630 (www.drugstore.com). Using cost analysis to adjust for the overall fivefold decrease in incidence with use, the number of children needed to treat to prevent 1 kidney stone is 11.7, hence $7371 worth of Polycitra K. The cost for treatment of an individual child with a kidney stone, including pediatric urology clinic visits, renal/bladder ultrasounds, urinalysis, and analgesic medications is approximately $2100. If necessary, the costs of admission for intravenous fluids, pain management, and time off work and school would then approach the combined cost of preventive oral potassium citrates. Although not always required, lithotripsy would cost an additional $7950 at our institution.
As would be predicted theoretically, urine pH within the empiric group was significantly higher than in the reactive group and identical to baseline values before the KD. This was even higher than the median urine pH of 6.0 reported before 2000, before oral citrates were first prescribed. Alternatively, serum CO2 was not different between the groups, suggesting that Polycitra K may not affect serum acidosis and its therapeutic effects may be primarily attributable to urine alkalosis. A future potential method to monitor compliance with Polycitra K might be to review urine pH data at KD clinic visits. In addition, possibly increasing the dosage if the urine pH is low (eg, <6.5) may be valuable in further avoiding kidney stones, as evident in the single child since 2006 with a stone despite Polycitra K and a urine pH of 6.0.

CONCLUSIONS

We strongly believe that universal use of Polycitra K is warranted for all children who start the KD, with clear benefits outweighing risks. We advise initial education about the importance of regular Polycitra K use at the time of KD onset and subsequent reinforcement at each follow-up clinic. Additional studies may help to determine whether higher dosages of Polycitra K would be completely preventive or could perhaps be combined with other supplements such as multivitamins to aid compliance.

High-Fat Ketogenic Diet Misunderstood, Underused


Two Johns Hopkins Children’s Center neurologists say persistent misunderstandings among physicians and patients about the high-fat ketogenic diet may have led to its underuse and are urging both its wider application for seizure disorders and further research into its therapeutic promise for brain cancer, Alzheimer’s and other diseases.
The opinion is contained in a review article in the journal Advances in Pediatrics by pediatric neurologists John Freeman, M.D., and Eric Kossoff, M.D.
 “Several persistent myths and half-truths seem to discourage both doctors and patients from trying a diet that, in many children, is more effective than medication and has fewer cognitive side effects,” says Freeman, professor emeritus of pediatrics and neurology at The Johns Hopkins School of Medicine.
The ketogenic diet has been used since 1921 to successfully treat several forms of epilepsy in patients who don’t respond to anti-seizure medications. Made up of high-fat foods and very few carbohydrates, the diet works by triggering biochemical changes that eliminate seizure-causing short circuits in the brain’s signaling system. Even though it has been used primarily in children, the diet has shown therapeutic promise in adults with epilepsy.
Anecdotal evidence also suggests the diet may be useful in the treatment of brain tumors, Alzheimer’s, stroke and Parkinson’s disease, and several human and animal studies have found tumor reduction after treatment with the ketogenic diet. The suspected mechanism is that cancer cells require glucose for survival, and the ketogenic diet deprives them of this essential fuel. One report described two children with inoperable brain cancer, whose tumors shrank remarkably after treatment with the ketogenic diet, leading to long-term survival of both patients. This evidence indicates that the diet may affect the body’s chemistry on a far more basic level than once believed, with effects far more profound than now thought, and requires further research, the authors say.
What has been well-established is that when it comes to seizures, the diet’s therapeutic effects are often longer lasting than those of medication, and it has resulted in a full cure of seizures in at least some children, the researchers say. Yet, the diet has remained somewhat misunderstood and underused, they say.
The most common misconceptions about the diet are that it causes dangerous biochemical changes, is inappropriate for use in infants and very young children, causes weight gain and fatty build-up in the arteries or that it should be a treatment of last resort. All of these, the authors note, are partly or entirely false.
The diet does require rigorous monitoring by a neurologist and a dietitian, which also may discourage patients and health-care providers from using it.
Several studies have documented the effectiveness of the ketogenic diet in reducing or completely eliminating seizures. In a 2001 Hopkins study, 75 out of 83 children who were on the diet for a year experienced full resolution or partial reduction of seizures. Many of them continued to do well even years after stopping the diet. A 2009 study in the journal Lancet reported nearly 40 percent (28) in a group of 54 children experienced greater than 50 percent reduction in the number of seizures on the diet. Five of the 54 had more than 90 percent fewer seizures after starting the diet. These were all children with drug-resistant epilepsy, whose seizures had not improved with two medications.
The diet is not without side effects, Kossoff and Freeman caution. These include vomiting and constipation, and a temporary moderate spike in cholesterol, which goes down after one to two years on the diet and returns to normal once the diet is stopped. A Hopkins Children’s study showed that one in 20 children on the diet develops kidney stones, but most cases can be prevented or treated with the supplement potassium citrate.
Kossoff and Freeman recommend the diet as a first-line treatment in two rare metabolic conditions — GLUT-1 deficiency and pyruvate dehydrogenase deficiency, both marked by seizures and other neurological problems. The diet, they say, may be a good earlier option for treating infantile spasms,myoclonic seizures, tuberous sclerosis, myoclonic-static epilepsy, Dravet syndrome, Rett syndrome and Lennox-Gastaut syndrome, all of which traditionally do not respond well to anti-seizure medication. In other seizure disorders, the diet should be considered after two medications have failed to resolve the seizures.

Κετογόνος Δίαιτα (Ketogenic Diet) ή Κετογονική ή Κετογενική ή Κετογενής ή Κετονική Δίαιτα


Κετογενική Δίαια (ή Κετογονική ή Κετογενής ή ΚετογόνοςΔίαιτα) είναι μια δίαιτα πλούσια σε λιπαρά, επαρκής σε πρωτεΐνες και φτωχή σε υδατάνθρακες, η οποία έχει σχεδιαστεί για να μιμείται πολλές από τις βιοχημικές μεταβολές που συνδέονται με την παρατεταμένη νηστεία. Η Κενογενική Δίαιτα αναπτύχθηκε αρχικά στις αρχές του 1900 και κατά τις δεκαετίες του 1920 του 1930 χρησιμοποιήθηκε με επιτυχία για τη θεραπεία επιληπτικών κρίσεων σε παιδιά. Όμως στη συνέχεια ξεχάστηκε σιγά-σιγά καθώς αναπτύχθηκαν νέα φάρμακα κατά της επιληψίας. Πρόσφατα ξαναήλθε στην επιφάνεια και χρησιμοποιείται σε όλο και μεγαλύτερη έκταση. Σήμερα, επαναπροσδιορίζεται ο ρόλος της ως εναλλακτικής θεραπείας για παιδιά με επιληψία που είναι δύσκολο να ελεγχθεί.

Η Κετογενική Δίαιτα δεν είναι μια δίαιτα της «μόδας» ή «κομπογιαννίτικη», αλλά μια εναλλακτική ιατρική θεραπεία για παιδιά με φαρμακο-ανθεκτική επιληψία. Η δίαιτα αυτή πρέπει να χρησιμοποιείται μόνο υπό ιατρική παρακολούθηση και με την ευθύνη διαιτολόγου.
1. Ιστορικό

Η νηστεία ως μέσο ελέγχου των επιληπτικών κρίσεων περιγράφεται στην Αγία Γραφή και αναφέρεται κατά τη διάρκεια του Μεσαίωνα. Στις αρχές της δεκαετίας του 1920 εμφανίστηκαν επιστημονικές ανακοινώσεις που περιγράφουν τις επωφελείς επιπτώσεις της παρατεταμένης νηστείας σε παιδιά των οποίων η επιληψία δεν μπορούσε να τεθεί υπό έλεγχο με τα νέα φάρμακα που κυκλοφόρησαν εκείνη την εποχή. Στις εν λόγω ανακοινώσεις υποστηριζόταν ότι η νηστεία, δηλαδή η κατανάλωση μόνο νερού επί 10 ως 20 ή και παραπάνω μέρες, μπορούσε να έχει ως αποτέλεσμα τον περιορισμό των επιληπτικών κρίσεων για παρατεταμένα χρονικά διαστήματα.

Εκείνη την εποχή, που μελετήθηκαν και οι επιπτώσεις που έχει στον μεταβολισμό ο σακχαρώδης διαβήτης, παρατηρήθηκε ότι ήταν δυνατόν να γίνει μια απομίμηση των βιοχημικών επιπτώσεων της νηστείας με μια δίαιτα πλούσια σε λιπαρά, αλλά με ποσότητες υδατανθράκων ανεπαρκείς για την πλήρη καύση των λιπών. Τα κατάλοιπα της ατελούς καύσης των λιπών είναι κετόνες (ή οξόνες) που εμφανίζονται στο αίμα. Σε πολλές επιστημονικές ανακοινώσεις από κλινικές έρευνες κατά τη διάρκεια της δεκαετίας του 1920 και του 1930 αναφέρθηκε ότι περίπου στο ένα τρίτο των παιδιών που ακολουθούσαν αυτή την Κετογόνο Δίαιτα, επιτεύχθηκε εκτεταμένη αναστολή των επιληπτικών κρίσεων, στο ένα τρίτο παρατηρήθηκε σημαντική βελτίωση όσον αφορά τον έλεγχο των κρίσεων, ενώ το ένα τρίτο δεν είδε κανένα όφελος. Ακόμη και σήμερα, οι μηχανισμοί με τους οποίους οι κετόνες, κυρίως το β-υδροξυβουτυρικό οξύ, περιορίζουν ή ακόμη και καταστέλλουν πλήρως τις επιληπτικές κρίσεις στα παιδιά, παραμένουν άγνωστοι.

Μετά την ανακάλυψη της phenytoin το 1938, το ενδιαφέρον για την Κετογενική Δίαιτα ατόνησε και στράφηκε στην ανάπτυξη νέων αντισπασμωδικών φαρμάκων. Καθώς αναπτύσσονταν νέα φάρμακα, η κετονογόνος δίαιτα χρησιμοποιούνταν όλο και λιγότερο και επειδή όλο και λιγότεροι διαιτολόγοι εκπαιδεύονταν στις λεπτομέρειες της δίαιτας, η χρήση της περιορίστηκε ακόμη περισσότερο και συχνά συνοδευόταν από αποτυχίες.

Το 1994 η πετυχημένη θεραπεία ενός παιδιού με επιληπτικές κρίσεις που ήταν δύσκολο να ελεγχθούν γνώρισε μεγάλη δημοσιότητα και αναζωπύρωσε το ενδιαφέρον για τη δίαιτα αυτή στις Ηνωμένες Πολιτείες. 
2. Τι είναι η Κετογενική Δίαιτα;
Όπως αναφέρθηκε και παραπάνω, η Κετογόνος Δίαιτα είναι μια δίαιτα πλούσια σε λιπαρά, επαρκής σε πρωτεΐνες και πολύ φτωχή σε υδατάνθρακες, η οποία έχει υπολογιστεί προσεκτικά και ατομικά για κάθε παιδί ξεχωριστά. Οι προσφερόμενες θερμίδες είναι περιορισμένες και εξαρτώνται από την ηλικία και τις δραστηριότητες του παιδιού. Αν υπολογιστούν σωστά, το παιδί δεν πρέπει ούτε να πάρει ούτε να χάσει σημαντικό βάρος, αλλά θα αναπτυχθεί κανονικά για τον σωματικό του τύπο και το βάρος του θα παραμείνει κοντά στο ιδανικό βάρος με βάση το ύψος του. Αν το παιδί είναι υπέρβαρο, οι θερμίδες περιορίζονται μέχρι να φθάσει κοντά στο ιδανικό βάρος. Περιορίζονται επίσης και τα προσλαμβανόμενα υγρά, αν και αυτό γίνεται για λόγους που είναι λιγότερο σαφείς. Με τη δίαιτα καλύπτεται το 90% περίπου των αναγκών του παιδιού σε θερμίδες υπό τη μορφή λιπαρών (κρέμα γάλακτος, βούτυρο, μαγιονέζα), ένα γραμμάριο ανά κιλό βάρους με τη μορφή πρωτεΐνης και ελάχιστη ποσότητα υδατανθράκων. Η Δίαιτα συμπληρώνεται με τη λήψη βιταμινών και ασβεστίου.
3. Τι επιτυγχάνει η Κετογενική Δίαιτα;
Η Κετογενική Δίαιτα έχει σχεδιαστεί για να προσομοιώνει πολλές από τις μεταβολικές επιπτώσεις της νηστείας. Όταν νηστεύουμε, το σώμα μας χρησιμοποιεί καταρχάς το απόθεμα γλυκόζης και γλυκογόνου και στη συνέχεια αρχίζει να καίει το λίπος που έχει αποθηκεύσει. Όταν δεν διατίθεται επαρκής ποσότητα γλυκόζης, τα λίπη δεν μπορούν να καούν πλήρως και από την ατελή τους καύση παραμένουν ως κατάλοιπα ουσίες που ονομάζονται κετονικά (ή οξονικά) σώματα (ακετοξικές και β-υδρόξυβουτυρικές ενώσεις). Με την Κετογενική Δίαιτα παρέχονται στο σώμα προς καύση εξωγενή λίπη (δηλαδή λίπη εκτός του σώματος) αλλά περιορίζονται οι διαθέσιμοι υδατάνθρακες, έτσι ώστε να αυξάνεται η συγκέντρωση των κετονικών σωμάτων. Φαίνεται πως αυτή η υψηλή συγκέντρωση κετονών προκαλεί την αναστολή των επιληπτικών κρίσεων.
4. Και τι τρώνε λοιπόν αυτά τα παιδιά;
Στα παιδιά επιτρέπεται να τρώνε μόνον τα προκαθορισμένα, προσεκτικά υπολογισμένα γεύματα. Παρόλο που οι μερίδες είναι μικρές, η παρουσία μεγάλων ποσοτήτων κετονικών σωμάτων στο σώμα μειώνει την όρεξη και τη δίψα και ύστερα από μια μικρή περίοδο προσαρμογής τα παιδιά σπάνια πεινάνε. Και παρόλο που η πιο κοινή αντίδραση στην ιδέα μιας τέτοιας πλούσιας σε λιπαρά δίαιτας τόσο από τους γιατρούς όσο και από τους άλλους είναι ένα αίσθημα «αηδίας», τα καλομαγειρεμένα κετογενικά γεύματα μπορεί να είναι εύγευστα και, ταυτόχρονα, ελκυστικά.

Το τυπικό πρωινό μπορεί, λόγου χάρη, να περιλαμβάνει μια ομελέτα με μανιτάρια και αρκετές φέτες μπέικον και ζεστή σοκολάτα από κρέμα γάλακτος με 36% λιπαρά. Το γεύμα μπορεί να αποτελείται από βλαστούς σέλινου γεμισμένους με φυστικοβούτυρο ή κρεμώδες τυρί, μαρούλι με μαγιονέζα και μια φέτα ντομάτας, καθώς και 
αεριούχο ποτό χωρίς ζάχαρη ούτε καφεΐνη. Το δείπνο, τέλος, μπορεί να έχει προσεκτικά ζυγισμένο λουκάνικο με σάλτσα ντομάτας ή μουστάρδα, μαρούλι με μαγιονέζα, μια παγωμένη γρανίτα με σαντιγί και μια φράουλα, καθώς και αεριούχο ποτό χωρίς ζάχαρη.
5. Πόσο αποτελεσματική είναι η Κετογενική Δίαιτα;

Παρόλο που η αποτελεσματικότητα της Δίαιτας έχει τεκμηριωθεί σε πολλές παλαιότερες μελέτες παραμένει εξίσου αποτελεσματική σήμερα σε παιδιά με δύσκολα ελεγχόμενες επιληπτικές κρίσεις και αυτό παρά την υιοθέτηση πολλών νέων αντισπασμωδικών (αντιεπιληπτικών φαρμάκων). Σε μια νεότερη μελέτη του νοσοκομείου John Hopkins (δημοσιεύτηκε στο περιοδικό Pediatrics το Δεκέμβριο του 1998) αξιολογήθηκε σε ομάδα 150 παιδιών η ανοχή στη δίαιτα και η αποτελεσματικότητά της.

Τα παιδιά αυτά είχαν, κατά μέσο όρο, πάνω από 400 επιληπτικές κρίσεις κάθε μήνα πριν αρχίσουν τη δίαιτα και είχαν δοκιμάσει κατά μέσο όρο πάνω από έξι διαφορετικά αντισπασμωδικά (αντιεπιληπτικά φάρμακα). Τα αποτελέσματα εκτίθενται στον παρακάτω πίνακα:

Πρέπει να σημειωθεί ότι το 71% από τα παιδιά που άρχισαν τη Δίαιτα, τη συνέχισαν για πάνω από ένα εξάμηνο και πάνω από τα μισά τη συνέχισαν για ένα χρόνο. Στις περιπτώσεις που η δίαιτα ήταν αποτελεσματική στην μείωση των κρίσεων, τα παιδιά την ανέχονταν καλά, και ο συχνότερος λόγος διακοπής της ήταν ότι δεν ήταν αρκετά αποτελεσματική για να δικαιολογούνται οι επιβαλλόμενοι περιορισμοί. Η Δίαιταήταν εξίσου αποτελεσματική σε διάφορες ηλικίες και σε παιδιά με διάφορους τύπους επιληψίας.
5.1 Συχνές ερωτήσεις σχετικά με τη Δίαιτα

5.1.1 Ποιος πρέπει να επιχειρήσει τη Δίαιτα;

Η Κετογενική Δίαιτα είναι μια εναλλακτική θεραπεία για παιδιά με επιληψία που είναι δύσκολο να ελεγχθεί. Η επιληψία που είναι δύσκολο να ελεγχθεί ορίζεται ως η επιληψία κατά την οποία σημειώνονται πάνω από δύο κρίσεις εβδομαδιαίως, παρόλη τη σωστή χρήση δύο και πλέον αντισπασμωδικών φαρμάκων. Η Δίαιτα δεν έχει επαρκώς μελετηθεί σε ενήλικες ή σε παιδιά ηλικίας κάτω του ενός έτους. Οι γονείς που νομίζουν ότι η Δίαιτα μπορεί να είναι κατάλληλη για τα παιδιά τους πρέπει να ζητήσουν ιατρική συμβουλή. Δεν πρέπει ποτέ να επιχειρήσετε να κάνετε τη Δίαιτα χωρίς ιατρική παρακολούθηση.
5.1.2 Θα μπορέσει το παιδί μου να σταματήσει να παίρνει αντισπασμωδικά φάρμακα;
Στόχος μας είναι όλα τα παιδιά που ακολουθούν τη Δίαιτα να απαλλαγούν τόσο από τις κρίσεις, όσο και από τα φάρμακα. Δυστυχώς αυτό δεν είναι πάντα δυνατόν. Παρόλο που μερικοί ασθενείς, οι οποίοι υποβάλλονται στη δίαιτα ενδέχεται να σταματήσουν βαθμιαία τα φάρμακα, πολλοί δεν απαλλάσσονται τελείως από τις κρίσεις, ενώ άλλοι χρειάζεται να συνεχίσουν κάποιου είδους φαρμακευτική αγωγή.

5.1.3 Μπορεί το παιδί μου να πάρει κάποια άλλα φάρμακα ενώ ακολουθεί τη Δίαιτα; Τι ισχύει για τα αντιβιοτικά;

Πολλά από τα φάρμακα για παιδιά χορηγούνται υπό τη μορφή σιροπιού που περιέχει ζάχαρη. Συνιστούμε στους γονείς να επαγρυπνούν και να διαβάζουν τις ετικέτες όλων των φαρμάκων και να αποφεύγουν όλα τα παρασκευάσματα που περιέχουν υδατάνθρακες. Στους υδατάνθρακες περιλαμβάνονται όλες οι ουσίες των οποίων οι ονομασίες λήγουν σε –όλη ή –όζη. Η Δίαιτα είναι συχνά πολύ ευαίσθητη, ακόμη και σε μικρές ποσότητες αυτών των υδατανθράκων. Για παράδειγμα, έχουμε διαπιστώσει ότι η αντηλιακή λοσιόν με βάση την σορβιτόλη (σημειώστε την κατάληξη –όλη) μπορεί να απορροφηθεί μέσω του δέρματος, μειώνοντας τη συγκέντρωση της κετόνης στο αίμα, με αποτέλεσμα να σημειώνονται κρίσεις.

5.1.4 Τι τρώει το παιδί στα γενέθλια; Στις γιορτές; Τι πρόχειρο φαγητό τρώει στο σχολείο;

Οι γονείς αναπτύσσουν στο έπακρο την εφευρετικότητά τους. Οι τούρτες γενεθλίων μπορούν να είναι από τυρί. Στις γιορτές μια οικογένεια επέτρεψε στο παιδί να πει τα κάλαντα και να μαζέψει ζαχαρωτά, που στη συνέχεια τα εξαγόρασαν και με τα λεφτά το παιδί αγόρασε ένα παιχνίδι. Κάτι τέτοιο μπορεί επίσης να γίνει στα πάρτι και στο σχολείο. Παιδάκια ακόμη και τεσσάρων χρόνων έμαθαν να λένε «Όχι ευχαριστώ, κάνω μια μαγική δίαιτα».
5.1.5 Μήπως αυτή η πλούσια σε λιπαρά δίαιτα προκαλεί αρτηριοσκλήρωση; (Σκλήρυνση των αρτηριών)
Μελέτες που βρίσκονται σε εξέλιξη υποδεικνύουν ότι μόνον σε ένα μικρό ποσοστό από τα παιδιά που υποβάλλονται στη δίαιτα παρουσιάζεται σημαντική αύξηση στη χοληστερίνη και στα τριγλυκερίδια. Οι μακροπρόθεσμες επιπτώσεις της Δίαιτας στις καρδιοπάθειες και στα εγκεφαλικά είναι άγνωστες, αλλά λίγα μόνο παιδιά συνεχίζουν να υποβάλλονται στη Δίαιτα για τόσο μεγάλο χρονικό διάστημα, ούτως ώστε κάτι τέτοιο να αποτελέσει πρόβλημα.
5.1.6 Μήπως τα παιδιά παίρνουν βάρος με τη Δίαιτα;
Όχι, κανονικά το βάρος τους δεν πρέπει να αυξηθεί πολύ. Η ποσότητα των θερμίδων προσαρμόζεται προσεκτικά στις ανάγκες του παιδιού. Αν το βάρος του παιδιού αυξάνει υπερβολικά, τότε μειώνουμε τις θερμίδες. Τα παιδιά δείχνουν να μεγαλώνουν κανονικά και το βάρος τους πρέπει να αυξάνεται μόνο κατ’ αναλογία με την αύξηση του ύψους τους.
5.1.7 Για πόσο καιρό πρέπει τα παιδιά να ακολουθήσουν στη Δίαιτα;
Τα παιδιά των οποίων οι επιληπτικές κρίσεις ελέγχθηκαν απόλυτα με την παρούσα Δίαιτα συχνά την σταμάτησαν, αφού επί δύο χρόνια παρέμειναν χωρίς κρίσεις. Άλλα συνεχίζουν τη δίαιτα για όσο καιρό το θεωρούν αναγκαίο. Αν σταματήσει η Δίαιτα ή αν μειωθεί το ποσοστό των λιπαρών και ξαναρχίσουν οι κρίσεις, τότε η δίαιτα μπορεί να ξαναρχίσει ή ο ασθενής μπορεί να ξαναϋποβληθεί σε φαρμακευτική αγωγή.
5.1.8 Πως λειτουργεί η Δίαιτα;
Φαίνεται ότι η κατανάλωση μεγάλης ποσότητας λιπών, σε συνδυασμό με τη μείωση των προσλαμβανόμενων υδατανθράκων και θερμίδων, προκαλεί κετοναιμία δηλαδή αύξηση της παρουσίας κετονικών (ή οξονικών) ενώσεων στο σώμα. Η σημερινή συνήθης ιατρική πρακτική απαιτεί οι κετόνες στα ούρα να παραμένουν σε επίπεδα 4+ (160-180mml). Πρόσφατα στοιχεία δείχνουν ότι η συγκέντρωση των κετονών στον ορό του αίματος μπορεί να είναι πολύ πιο κρίσιμη από τη συγκέντρωσή στα ούρα. Ενώ μπορεί να απαιτείται συγκέντρωση 4+ στα ούρα για τον έλεγχο των κρίσεων, αυτό μπορεί να μην επαρκεί για τον βέλτιστο έλεγχο και να πρέπει να μειωθούν περαιτέρω οι θερμίδες ή να αυξηθεί ο λόγος των λιπαρών προς τις πρωτεΐνες και τους υδατάνθρακες. Βρίσκονται σε εξέλιξη μελέτες των συγκεντρώσεων των κετονών στον ορό του αίματος και της σχέσης τους με τον έλεγχο των επιληπτικών κρίσεων.

Δεν είναι γνωστός ο μηχανισμός με τον οποίο τα κετονικά σώματα καταστέλλουν τις επιληπτικές κρίσεις και προς το παρόν αυτό αποτελεί αντικείμενο έρευνας. Ωστόσο, η μείωση της παρουσίας κετονικών σωμάτων μέσω της χορήγησης υδατανθράκων, μπορεί να οδηγήσει γρήγορα στην επανεμφάνιση των κρίσεων.

6. 
Μελλοντικά πεδία για έρευνα

Η επιτυχία της Δίαιτας να μειώνει και μερικές φορές να σταματά τελείως δύσκολα ελεγχόμενες επιληπτικές κρίσεις, αναζωπύρωσε το ενδιαφέρον για τον τρόπο με τον οποίο επενεργεί η δίαιτα, και ορισμένοι ερευνητές άρχισαν να χρησιμοποιούν πειραματόζωα για να τη μελετήσουν. Χρησιμοποιώντας πειραματόζωα θα καταστεί δυνατό να μελετήσουμε τον τρόπο με τον οποίο τα κετονικά σώματα μεταβάλλουν τα κατώφλια των επιληπτικών κρίσεων και να κατανοήσουμε τους μηχανισμούς με τους οποίους η δίαιτα καταστέλλει τις κρίσεις. Ενδεχομένως, όταν θα κατανοήσουμε τους μηχανισμούς, ίσως να είμαστε σε θέση να αναπτύξουμε κάποιο φάρμακο που να προσομοιώνει τις επιπτώσεις της δίαιτας.
Οι κλινικές έρευνες ίσως οδηγήσουν σε αλλαγή του τρόπου με τον οποίο ακολουθείται η δίαιτα. Όταν θα μπορέσουμε να παρακολουθούμε τις συγκεντρώσεις των κετονικών σωμάτων στο αίμα και όχι μόνο στα ούρα, ίσως να μπορέσουμε να προσαρμόσουμε τη Δίαιτα έτσι ώστε να πετύχουμε τη μεγιστοποίηση της συγκέντρωσης των κετονικών σωμάτων. Ίσως καταλάβουμε ότι μεγαλύτερες συγκεντρώσεις κετονικών σωμάτων οδηγούν σε καλύτερο έλεγχο των κρίσεων. Μπορεί να μάθουμε πώς να τροποποιούμε τη Δίαιτα έτσι ώστε να μειώσουμε τον σχηματισμό λίθων στα νεφρά. Και θα μάθουμε επίσης περισσότερα για τις επιπτώσεις της Δίαιτας στα λιπίδια του αίματος, καθώς και για τις μακροπρόθεσμες επιπτώσεις της Δίαιτας. Ας ελπίσουμε ότι όλες αυτές οι μελλοντικές προσπάθειες θα οδηγήσουν στον καλύτερο έλεγχο των επιληπτικών κρίσεων στα παιδιά εκείνα που πάσχουν από επιληψία της οποίας ο έλεγχος είναι δύσκολος.
